Commonly Requested Forms

  • Federal W4 tax withholding
    You must complete this form to set the number of deductions and additional withholding amount, if applicable, when you begin employment with the University. You may also adjust your federal withholding at any time throughout your employment.
  • Maine W4 tax withholding
    You must complete this form to set the number of deductions and additional withholding amount, if applicable, when you begin employment with the University. You may also adjust your state withholding at any time throughout your employment.
  • Direct Deposit request
    All payroll payments are made through direct deposit. You may use this form to establish or update your requested direct deposit request.
  • Employee Information form
    You may use this form to make changes to your emergency contact information, update your home address, or update your UNE campus address.
  • Tuition Grant in Aid form
    Full-time and half-time employees may take courses at UNE at reduced rates. You may complete this form and return it to Human Resources to apply for the discount after you register for the course. Please refer to the Personnel Handbook for policy details and limitations.
  • CIC Tuition Grant in Aid form
    Full-time employees and their dependents may apply for tuition exchange benefits at other Council of Independent College institutions. To participate in this program, you need to complete and submit this form.
