This presentation offers essential insights for future healthcare providers to empower teenagers with disabilities to take charge of their health as they mature. We'll discuss practical strategies for fostering independence, understanding the shift from pediatric to adult healthcare models, managing complex care needs, and ensuring a seamless transition to the adult system. Join us to equip yourself with the tools needed to support a healthy and independent future for your young adult. Participation is applied toward students’ IP Honors Distinction Application.
