Understanding Your Billing and Insurance Responsibilities
Your health insurance will be billed for services provided at the Student Health Center. You are responsible for providing Student Health Services with current, valid health insurance information at your first visit. You must provide us with updates if there are any changes at any time to your health insurance. If you have an HMO, you are responsible for obtaining a referral from your PCP for our services prior to your appointment with us.
It is your responsibility to understand which services your insurance covers and which it does not. You are responsible for any co-payment or co-insurance required by your insurance. If you have questions about your coverage, you can call the number on the back of your insurance card, and they will assist you.
Charges from other facilities, such as laboratory fees, hospital care, X-rays, and specialist visits, are your responsibility. You will be billed separately for these charges.
If the services you receive at Student Health are not covered, or are not fully covered, by your insurance, you are responsible for any remaining balance. Bills will be sent to the address on file in Medicat. We are not responsible for statements, explanations of benefits, or other communications sent to your home address from your insurance company or outside facilities.
